Make an electronic copy to share with me when completed.\nInformation entered should include accurate spelling and correct DOBs for all participants (including coaches) are required.\nIncomplete forms (missing DOBs, events, qualifying scores, etc.) will not be accepted.\nUse one line per delegation member and be consistent with your delegation’s name.\nDo not modify the format of the spreadsheet.\nONLY 3 events per athlete are allowed, with exception of the 4×100 Unified or Special Olympic Relay Teams, which can be added to the 3 events. Relay teams require 4 participants.\n\nRequired Paperwork\nAll required paperwork must be completed prior to the event for Athletes, Coaches, Volunteers, and Unified Partners. Please note that Volunteer A and Unified Partner forms expire every three years.\n\nAthlete Registration / Renewal Form – Special Olympics Maine now uses an annual Renewal Form for returning athletes.
